How to apply
The housing referral process
A clear, accountable path from inquiry to stable placement. PHN does not guarantee housing or a specific timeline — availability and occupancy are confirmed individually per property.
The steps
From inquiry to placement
01
Inquiry
A resident, family member or referring professional submits a housing inquiry through PHN.
02
Intake & Needs
PHN gathers needs, preferences and any accommodation requests to match appropriately.
03
Matching
We match the person with a verified operator or property in an active market.
04
Referral & Follow-Up
A referral is made and PHN follows up to support a stable placement.
What we ask on the public form
- First name and preferred contact method
- Phone or email
- City desired, budget range, move-in timeframe
- Private/shared room preference and transportation needs
- Consent to follow-up and privacy acknowledgment
What we never ask publicly
- Social Security numbers
- Diagnoses or medications
- Discharge papers
- Detailed criminal records
If a later application review requires sensitive documentation, we use a secure, separately authorized process — never a public marketing form.
